Adirondack Region Medical Home Pilot
On January 1, 2010, primary care physicians at CVPH began participating in an exciting new model for the delivery of health care services. The Adirondack Region Medical Home Pilot is designed to more appropriately reimburse primary care physicians while facilitating their ability to provide the comprehensive medical care that will benefit their patients and ultimately conserve resources. This pilot is the largest in the United States, both from a geographic standpoint as well as the number of diverse providers involved. The project runs from the Canadian border 120 miles south, and from Lake Champlain to approximately 60 miles west and involves approximately 100 physicians and five hospitals.
The Medical Home encourages an emphasis on preventive care, the management of chronic disease, and overall coordination of services to achieve the best outcomes. All major insurance carriers in the region including New York State Medicaid support and endorse the pilot, which has also resulted in a significant investment in electronic medical record capabilities, further enhancing physicians' ability to practice.
