Located in Indianapolis, Indiana, Community Health Network is seeking a BE/BC Family Medicine or Internal Medicine Physician to become the Medical Director of our Hospice and Palliative Medicine Program.
This position will involve developing our Hospice and Palliative Medicine fellowship program and also working in our Life’s Journey program. At this time Life’s Journey, our chronic disease management program, is primarily acute care for seniors, but eventually will also provide Pediatric Palliative Care. Other responsibilities of this Medical Directorship will include outpatient supervision and teaching of Hospice and Palliative Care fellows, curriculum development, staffing and grant projects. Ideal candidates will be interested in working within a multidiscipinary team and working closely with their patients families. Candidates should also have a consistent and consecutive employment or training record.
